If you are a new patient coming to one of PPWP’s family planning centers for a gynecological examination,
to obtain a birth control method, or for sexually transmitted disease testing or treatment, you may download
the history form and bring it with you to your visit, thus saving some time in the waiting room.
Abortion Services
If you are a new patient coming to Planned Parenthood of Western PA to
obtain an abortion, you may download the intake forms and bring them
with you to your visit. This will save some time in the waiting room and
make for a shorter visit for you.
If you were determined to be eligible for Justice Funding by an
appointment scheduler, you may download the fund survey (English Version | Spanish Version) and bring it
with you to your visit. This will save some time in the waiting room and
make for a shorter visit for you.
